The United States confirmed 2,318 measles cases by 24 July, 58 of them reported that day alone, passing the 2,289 recorded across the whole of 2025 1. On 9 July we reported the count at 2,231 and noted it was on pace to break the record . It broke it a fortnight later.
The Center for Infectious Disease Research and Policy (CIDRAP) at the University of Minnesota, citing the Johns Hopkins University measles tracker, calls this the worst year since 1991 2, and Dr Andrew Racine, president of the American Academy of Pediatrics, said on the record that "halfway into 2026, the U.S. is experiencing the worst year for measles since 1991" 3. That comparison reads better with its numbers attached. 1991 logged 2,126 cases, fewer than 2026 has already recorded, at the tail of a resurgence that peaked at 9,643 the year before 4. The claim concerns which era American measles has returned to, not which single annual total has been beaten.
Severity has moved the other way. No deaths have been recorded in 2026 against three last year, and 7% of cases have needed hospital care against 11% in 2025 5. Ninety-three per cent of patients were unvaccinated or of unknown vaccination status and 69% were under 19, a profile consistent with transmission concentrated among children in communities with low coverage but workable access to care. measles deaths lag cases by weeks, so a clean mortality column in July guarantees nothing about October.
Utah leads the state table on 514 cases, then Virginia on 177, Pennsylvania on 139, Arizona on 119 including a cluster traced to a business in Aguila, Maricopa County, and Washington on 46 6. Those are self-reported state figures. The map kept by the US Centers for Disease Control and Prevention shows 522 for Utah and 134 for Pennsylvania, a discrepancy CIDRAP flags itself, and a reminder that the national total is an assembly of fifty reporting systems rather than a single count.
