PAHO (the Pan American Health Organization), WHO's regional office for the Americas, published a measles table on 8 August that puts Guatemala at 30,371 confirmed cases and 26 deaths, the largest outbreak in the region 1. Mexico follows at 12,255 cases and 17 deaths, then the United States at 2,260, Peru at 1,277 and Canada at 1,107. Those five hold 95% of the region's 47,459 confirmed cases, against 15,011 for the whole of 2025. PAHO rates the regional risk very high.

Read the word confirmed twice before setting any two lines of that table against each other. A confirmed case is one a laboratory has tested and a national system has reported, which makes every count a product of two things at once: how many people caught measles, and how many of them a health ministry could reach, swab and process. Compare two countries' confirmed counts and you compare their laboratories as much as their epidemics. Guatemala's rise from the 6,209 we published on 17 June works out at 389%, close to fivefold in about a month, and a jump that steep can come from a cleared testing backlog or a widened case definition as readily as from transmission. Nothing PAHO published separates the two, which leaves the direction of Guatemala's outbreak certain and its slope unknown.

The table also closes at epidemiological week 28, which ended on 18 July, so an August publication describes a mid-July region. Whatever Guatemala's count is today, it is not 30,371.

On severity, Mexico has the argument. Guatemala's 26 deaths on 30,371 cases give a case-fatality ratio of 0.09%; Mexico's 17 on 12,255 give 0.14%, half again as high per patient. And the Mexican focus was never irrational: Mexico co-hosts the World Cup, and tournament travel carries an exportation risk that Guatemala's outbreak does not. PAHO's 7 August alert lists mass gatherings with broad international participation among general risk factors, alongside vaccination coverage and accumulated susceptibility, and attributes no case to the tournament 2.
